Community Health Worker Coaching Program
Overview:
Building on our Healthy Food Rx food is medicine intervention, which unites Stockton-based organizations to assist individuals with diabetes by providing home-delivery of free recipe-based food boxes to participants, our team collaborated with Community Health Workers (CHWs) to develop a six-month, standardized 1:1 health coaching program. This program is designed to support residents with diabetes by offering education and social support. Scripted lesson plans were created to enable CHWs to follow a structured curriculum, ensuring consistency and effectiveness. Additionally, clients received an action plan journal in their preferred language (English, Spanish, or Khmer) to reinforce the lessons and support their progress.
Impact:
CHWs, as trusted community members, provided effective health coaching, motivation, and support. Evaluation results showed significant improvements in all measured health behaviors, demonstrating that CHW-led coaching can drive healthy lifestyle changes among patients with diabetes.
