Strengthening statewide coordination to advance nutrition security and collaborative public health action across Colorado
Aligning Prevention Activities
The Colorado State Nutrition Action Council (SNAC) was established in federal fiscal year 2017 in response to USDA requirements to align nutrition and chronic disease prevention activities across Food and Nutrition Service programs. The Colorado Department of Human Services (CDHS) provides leadership and administrative support, fostering collaborative statewide action.
From FFY 2023 through FFY 2025, PHI Center for Wellness and Nutrition (PHI CWN) supported Colorado SNAC through coordination, facilitation, evaluation, and training services. The council serves as a statewide platform for policy, systems, and environmental (PSE) change efforts focused on improving food and nutrition security across Colorado communities.
Colorado SNAC brings together state agencies, nonprofits, universities, healthcare organizations, and community partners to strengthen collaboration, share best practices, and advance coordinated public health strategies.

Resource Hub
PHI CWN supported Colorado SNAC through the development of its first major initiative: a centralized Resource Hub designed to strengthen statewide collaboration and information sharing among SNAC partners. The hub provides templates, sample documents, meeting materials, and shared tools that support coordination, communication, and partnership development across organizations working to improve nutrition security throughout Colorado.
