We provide program coordination, training, technical assistance, data collection and evaluation for Food is Medicine projects building community-clinical connections to increase healthy food access, increase food security, prevent chronic disease, and promote health equity. 

LA County Grocery Voucher Program

The Public Health Institute’s Center for Wellness and Nutrition (PHI CWN) partnered with Los Angeles (LA) County’s Nutrition and Physical Activity Program and local Community Based Organizations to implement two variations of the LA County Grocery Voucher Program, which provides grocery benefits to low-income households not currently receiving food assistance such as CalFresh. 

The vouchers could be used online for food box delivery or in-store at participating grocers and aimed to improve nutrition security by increasing access to healthy foods. 

Healthy Food Rx Program

Abbott’s Healthy Food Rx program is designed to help manage diabetes by providing access to nutritious foods and education as part of Abbott’s Future Well Communities initiative, which aims to address the social and economic barriers to good health in the fight against diabetes. 

Participants receive a recipe-based food box filled with nutrient-rich ingredients delivered to their doorstep. Nutrition Educators also offer workshops and cooking classes, including live cooking demonstrations, to teach participants how to prepare the meals and understand the nutritional benefits of the ingredients.
